POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans in an acute rehab setting.

MINIMUM REQUIRED QUALIFICATIONS – CA RN license; BLS; 1+ year acute case manager experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Mon-Fri with a weekend rotation

SYSTEMS – Meditech

START DATE – ASAP
